Our goal is to provide direction and support to families that will improve their quality of life and ability to achieve self-sufficiency.  We promote this concept through each program and service we administer.

Area Five Agency’s mission statement is: To provide programs and services that will benefit directly or indirectly, low-income, disadvantaged, or aged persons.

Our vision statement is:  Enriching Lives and Improving Communities

Area Five Agency on Aging and Community Services, Inc. was formed on September 12, 1974 when Congress passed the Older American’s Act designating funds to specifically help older adults retain their dignity and independence.  As an Agency on Aging our senior programs strive to meet the needs of the older adults living within our communities while helping families deal with the changes that come with aging.  In 1979, Area Five Agency expanded to become a Community Action Agency.  At this point, our mission grew to include programs to benefit low-income persons and assist them in maintaining maximum independence and dignity, to remove social barriers to economic and personal independence of all people, and to encourage community awareness of the needs and potentials of families in crisis and in need of assistance.

Information and referral services are housed locally through our Aged and Disabled Resource Center (ADRC).  Some services include assisting individuals and families with enrollment services for health insurance, prescription drug programs, winter heating assistance, and nutritional programs for children and seniors.  Our Head Start and Healthy Families programs offer educational activities for parents and children, while our case management team provides options within home and community based services for eligible individuals.  For more details on specific programs or meeting a specific need, please contact us or visit our website.

Donations of cash, volunteer time, services, or supplies are one of the many ways to get involved with Area Five Agency on Aging & Community Services, Inc.  Best of all, your cash donations are tax deductible!

Area Five Agency is governed by a Board of Directors.  The board consists of members from public, private, and low-income sectors within the communities served.  They review programs and services offered by the agency monthly and establish long term strategic goals.

If you are in need of services for yourself or a loved one, please do not hesitate to give us call at (574) 722-4451 or 800-654-9421.  We are here to help connect people to resources for a healthier tomorrow.
