Kokomo, Logansport, Rochester Clinics-10:00 a.m. -12:00 p.m.
Winamac (Tuesday & Wednesday)-10:00 a.m. -12:00 p.m.
Tipton (Wednesday & Thursday)-10:00 a.m. -12:00 p.m.
- Requirements: Individuals seeking services should bring their photo identification and insurance coverage information. For those under the age of 18, a parent or legal guardian must accompany them.
- Expansion Plans: 4C Health plans to expand these efforts to provide no barrier, no wrong door access to consumers 24/7.
