Dementia Care Specialist will expand supportive services for dementia care and caregiver support. They collect data to help support the need of services and supportive services. Increasing public and community awareness of dementia, and clinical and community resources available to support people living with Dementia and their care partners, including early screening opportunities. Relationship-building, educating, and facilitating collaboration with other community-based organizations and healthcare providers to increase referrals to clinical, care coordination and community resources in support of people living with Dementia and their care partners.  Carry out the duties outlined in the grants and contracts. Strength in data collection and entry of all appropriate documentation in CAMSS, Excel and any other documentation programs. Participate in Area Five and State meetings. Familiarity with contracts, grants and budgets outlined. Education and/or experience:  Associate degree (A. A.) or equivalent from two-year college or technical school; or six months to one-year related experience and/or training; or equivalent combination of education and experience. The Expanding the Public Health Workforce within the Aging Network being a new grant experience with Case Management, working with disabled community, Dementia and Alzheimer’s experience plus.
