
4C Health celebrates one-year success of Rural Enhancement and Access for Consumers Experiencing Homelessness (REACH) grant

North Central, IN— 4C Health proudly marks the one-year success of its Rural
Enhancement and Access for Consumers Experiencing Homelessness (REACH) grant,
funded by the Substance Abuse and Mental Health Services Administration (SAMHSA). This
initiative has expanded outreach, improved access to behavioral health services, and
strengthened community connections for individuals experiencing homelessness across
Miami, Cass, and Howard counties.
Over the past year, the REACH program served 341 individuals experiencing homelessness
or housing instability, connecting them to behavioral health services, community
resources, and pathways to stability. County-level impact includes:
- Miami County: 32 individuals served
- Cass County: 71 individuals served
- Howard County: 238 individuals served
“The REACH grant has allowed 4C Health to meet individuals where they are—both
geographically and in their journey,” said Alanna Lease, Director of Adult Intensive Services.
“By reducing barriers to care and partnering closely with local organizations, we are able to
provide timely, compassionate support to some of our most vulnerable community
members.”
In addition to direct services, 4C Health executed three community outreach events in
Howard, Miami, and Cass counties, reaching 117 individuals. These events offered on-site
resources, education, and direct connections to behavioral health services, housing
supports, and recovery-focused care—strengthening awareness and access within each
community.
The Rural Enhancement and Access for Consumers Experiencing Homelessness (REACH)
program focuses on proactive engagement, early intervention, and collaboration with
community partners to ensure individuals experiencing homelessness in rural areas can
access the services they need. Through targeted outreach and person-centered care, the
program addresses gaps in service delivery while promoting long-term wellness and
stability.
As 4C Health looks ahead, the organization remains committed to building on the success
of REACH by expanding outreach efforts, deepening partnerships, and continuing to
address the evolving needs of individuals experiencing homelessness throughout the
region.
